RESUMO

. The resignations among nurses in the Veneto healthcare institutions. A retrospective study. INTRODUCTION: The phenomenon of large-scale resignations is complex, heterogeneous and cannot be attributed solely to the pandemic crisis, during which many people reconsidered the role of work in their lives. The health system was particularly exposed to the shocks caused by the pandemic. AIMS: To describe the turnover and analyse the resignations of nurses in the NHS hospitals and districts of the Veneto Region. METHODS: The hospitals were categorised into 4 types: Hub and Spoke of level 1 and 2. The positions of nurses with permanent contracts between 1 January 2016 and 31 December 2022 were analysed, considering active, nurses on duty for at least one day. The data were extracted from the human resource management database of the Region. Unexpected resignations were considered those occurring before the retirement date (59 years for women and 60 for men). Negative and overall turnover rates were calculated. RESULTS: The risk of unexpected resignations was increased for nurses not resident in Veneto, of male gender and employed at a Hub hospital. CONCLUSIONS: The flight from the NHS is in addition to the physiological flow of retirements, due to increase in the coming years. It is necessary to: act on the retention capacity and attractiveness of the profession; implement organisational models based on task sharing and shifting; implement digital tools; favour flexibility and mobility to improve the balance between work and personal life; efficiently integrate professionals quali-fied abroad.

. The education of specialized nurses' aides: current scenario and future perspectives. INTRODUCTION: The complementary training in health care of the nurse aides (NAs), launched in Italy in 2003, has little impact on health care organisations. OBJECTIVE: To analyse, through the regional provisions issued in the last three years, the current state of complementary training in health care and propose some recommendations for the nursing profession with respect to the inclusion of this figure in care teams. METHODS: A documentary content analysis was conducted with respect to six main topics: recipients and admission to the courses, organisation and characteristics of the courses, learning evaluation, responsibilities of the new NAs, expected competences and skills. RESULTS: There is a clear need to update and standardise the contents of the courses, with a focus on current social and health problems and future scenarios, and to render more homogeneous the structure of the courses, to guarantee high standards at national level. The need of increasing the skills of the NAs is associated with the implementation of innovative care and organisational models that require the ability to care for populations whose needs have changed profoundly over the last twenty years. CONCLUSIONS: Health care teams with professionals with different skill-mix guarantee safe, appropriate, effective and efficient care: the organization should enable nurses to free up time to act out their competences in care planning, health promotion, therapeutic education, transition management and specialist functions.

RESUMO

The professional profile of health social care worker (OSS) was introduced in Italy in 2001 and has become a fundamental figure in the Italian healthcare and social-health services system. However, changes have occurred in Italian health, clinical care and social realities since the OSS profile was defined. This article reports the results of a national-level study to investigate the state of application of the State-Regions Agreement of 22 February 2001, establishing the professional profile of OSS and regulating its characteristics and competencies, as that of the agreement of 16 January 2003, regulating the complementary healthcare training for nursing assistants.

RESUMO

Presented here is the approach used by a multidisciplinary working group fo the drafting of the "core" competence profile of the healthcare professions manager in the Veneto Region. Defining a competence profile allows for specifying a standard for measuring the skills acquired by a professional and the gap level from what is expected by the organization, as well as orienting the preparatory education to carry out the related role.

INTRODUCTION: The indicators used in the Italian National Outcome Strategy does not include measurement of nursing care outcomes so these have not yet been assessed systematically in our country but only in the context of specific research projects. Positive and negative outcomes of nursing care have been documented in the literature, the latter associated with missed nursing care, a phenomenon that occurs when conditions are such that nurses are unable to deliver planned care to patients. OBJECTIVES: To describe the rationale, methodology and main results achieved to date in the development of a regional policy aimed at establishing a panel of indicators for monitoring nursing care-sensitive outcomes hospitals in publicly funded hospitals in the Veneto Region (northern Italy). METHODS: A plurennial, multi-method project based on [1] identification of selection criteria for a Minimum Data Set of indicators; [2] a rapid review of the literature and of the policies established internationally to measure nursing care-sensitive outcomes; [3] the establishment of an initial panel of indicators and evaluation of any critical issues with the chosen indicators, and [4] identification of the most appropriate tool for measuring missed nursing care. RESULTS: The medical and surgical units were considered to be the most suitable settings for the pilot study. Following the literature review, indicators that were already being monitored in the current regional information system were chosen, with the intent to prevent an additional administrative burden to nurses. By using a progressive consensus process, five outcome indicators (functional status, falls, pressure sores, urinary tract infections, aspiration pneumonia) and one process indicator (missed nursing care) were selected., and the tools for measuring the above-mentioned indicators and their related risks, were identified. A regional policy was then established to measure these indicators in a pilot phase, with the intent of implementing them as stable indicators to be measured in the new computerized hospital information system. CONCLUSIONS: An initial panel of nursing-sensitive outcome indicators has been defined to be used in Internal Medicine and General Surgery units of hospitals in Italy's Veneto Region. Despite its limitations, the project represents the first effort to create a regional policy to measure the contribution of nursing care to the health outcomes of patients and that will also the identification of potential relations with other variables such as personnel staffing and/or skill mix.

RESUMO

The practice of assigning a tutor to nursing students is becoming more common. The aim of this study was to assess how nursing students perceived their tutors in the light of the personal and professional characteristics of both trainers and trainees. Nursing students were asked to anonymously fill in a questionnaire to evaluate how they perceived their tutors that included an Italian translation of the Nursing Students' Perceptions of Instructor Caring -Short Version (NSPIC). A total of 243 students , predominantly female (74%) with an average age of 24.6 years were included in the study. On the whole ,tutor evaluation was positive (average 146.3 on a scale ranging from 31-186) and results showed that the personal and professional characteristics of trainers and trainees are not very influential.

AIM: The prevalence of deafness in a pediatric hospital Caritas Baby Hospital in Bethlehem was described. METHOD: All the clinical records of neonates born September 25, 2006 to March 25, 2007 screened for deafness were selected. RESULTS: Among the 274 screened newborns, 112 were females and 162 males, 97% from the districts of Bethlehem and Hebron, and 3% from other locations in the West Bank. The most recurrent diagnoses were jaundice, potential sepsis and prematurity. Sixty-three were positive at the first screening, 26 at the second screening and only 4 were visited by the audiologist, who confirmed a hearing loss (medium or high) in 3 newborns. CONCLUSIONS: Screening for deafness should be extended to all newborns and parents should be educated increase awareness of potential problems of children and to refer the child to a specialist.
